I am an out of network provider with health insurance companies. As a courtesy to clients with health insurance, I will provide a monthly "Superbill" with all information needed so that you may submit this to your health insurance company. Your health insurance company will then send you a reimbursement check for whatever funds you are due (minus deductible, copay, and coinsurance).
